How to unlock the Tear 'n' Rip achievement in Serious Sam 4 - Definitive Guide
Quickest method to get this is running the "One for the Road" mission on "Tourist" difficulty. Ensure you have S.A.M perks "Healing Punch, Long Distance Relationship, The Bigger They Are and Harder They Fall" unlocked. Run up to the large "Scrapjack" at the beginning of the mission, headshot him once with the Shotgun (not the double barrel) and doing a melee takedown when he slumps over. I find this technique works every time on a Scrapjack on Tourist difficulty.

Works great. However, if anybody doesn't have enough skill points to get the last skill (I had 3 instead of 4), simply load up the same level in co-op and you should be fine (it gave me 6). Thanks for the solution!

For this achievement you will be using the Scrapjack enemy. The option to melee big enemies will be available after using skill points in the S.A.M. tree. (Press Y and open S.A.M.) The melee option is categorized in 3 levels.
Some tips:

S.A.M. (Skill tree):
1. Default melee: Small enemies can be melee-attacked from the very start. (Requires 0 skill points)
2. The Bigger They Are: Perform melee attacks on medium enemies. (Requires 3 skill points)
3. The Harder They Fall: Perform melee attacks on large enemies. (Requires 4 skill points)
For this achievement you need to be level 3. To get to level 3 you will be needing a total of 4 skill points. If you don't miss any S.A.M. skill point pick-ups the earliest you can get this is in mission 6: Morituri Te Salutant. For this achievement you want to make sure to be playing on at least Normal-difficulty. When you play on lower difficulties it will be very hard to get the melee prompt as you insta-kill enemies most of the time. When you find a scrapjack you need to use the double shotgun, run close to the scrapjack and shoot him in the face. If done correctly the scrapjack will fall on the ground and a melee-attack prompt (Press RS) will show up. After performing the melee-attack the achievement will unlock.2. The Bigger They Are: Perform melee attacks on medium enemies. (Requires 3 skill points)
3. The Harder They Fall: Perform melee attacks on large enemies. (Requires 4 skill points)
Some tips:
1) If you missed out on S.A.M. skill point pick-ups during your solo playthrough you can always use level select in Co-op mode. Select mission 7 or a later mission and you will have at least 4 unused skill points. (You can only replay a mission in Co-op mode if you previously completed it in your solo playthrough)
2) If you correctly used the skill points in your solo playthrough you can manually save before attempting the melee attack, this is a big advantage over the Co-op mode way, as you can't save over there. (If you are playing on Tourist or Easy difficulty you can temporary change to Normal difficulty)

Keep collecting S.A.Ms until you get to the point where you can on your skill tree buy the ability to melee kill big enemies, once you can do this the easiest enemy to do it on will be the fatties with the giant guns on either arm. Simply go up to its face on any difficulty (I did it on easy) with the double barrel equipped, shoot it straight in the head for it to get stunned and then when it falls with your gun out, press in the right thumbstick to melee finish it off.

doesnt work i tried to stun them with double barrel shotgun or akimbo double barrel shotgun and there is no melee option even tho i upgraded my meleeskills

It works best with a single double barrel round and it may not work every time as it can be finicky. Aim as close to the center of the head as possible and be at the front of the enemy. The headshot "Star" symbol should pop up when your round hits their face.

After dozens of attempts on all difficulties to do get this with the scrapjack-double barrel method and never getting the melee prompt, I found - what I think is - a better method.
Later in the game, you will get the cannonball. You can get the melee prompt if you shoot a cannonball directly at the head of a Khnum. In my case, I loaded up "Anathema unto God" on Easy difficulty and did exactly that to the first Khnum I saw and got the prompt on the first attempt.
