To get this achievement you must keep doing quests and challenges and when you unlock the 50th item you will get it.
You don't have to wear the items just unlock them.

You open clothing items by completing story missions, completing challenges and finding the treasure chests. You should get this about 1/3rd of the way in to the game.

You recieve different clothing rewards/unlocks for completing missions, and finding secret chests. This unlocked when I opened up my 3rd secret chest, but it could unlock at different times for each player.
