How to unlock the Yummy Meal achievement in Shenmue - Definitive Guide

You can get this at the start of the game.

After the intro cut scenes and leaving your room head straight to the kitchen. The table in front of you will have some food. Press LT and your character will focus on the table, if he doesn’t adjust your camera to look at it until it zooms in. Press cn_A and a short cutscene will play where Ryo is a boy. During the cinematic the achievement will unlock.

Thanks to Memoryslave here is some additional info.

If you don't get it right away at the beginning of the game and you find that there's nothing on the table, just wait until you can go to sleep and just after waking up head to the kitchen and food should be there.

The food is there but when I focus on it and press A nothing happens.

Worked for me several days later into the game. Then I checked the list of achievements and went for it.
F U to the clowns that gave this great solution a thumbs down. +1

Yeah I try every day. Still nothing. I'll just have to restart. I missed the yard flashback anyway

I would maybe let a day pass and try it again. I haven’t heard of this issue so I’m not sure of a sure fire way to fix it.

I had the same problem that Brave had where i would look at the food and nothing happened. I started a new game and went to the kitchen right away and it worked!

After your first day with Ryo, if you head into your kitchen at home you will see lunch laid out. Just look at it to trigger this flashback.

MISSABLE

Enter the kitchen behind you. There will be food on the table. Use to zoom in on the food. A short cut-scene will play and you will be rewarded with this trophy.

Trophy is easy to miss
At the very beginning, after we get control, we go to the kitchen. There is food on the table. Clamp and look at the plates. After watching the video, the trophy is yours
