This ending is quite difficult and will most likely not be obtained on a first playthrough. Here is a more extensive guide on how to get the Maria ending.   • Heal James like there's no tomorrow. Spending significant amounts of time at full health attribute greatly towards this ending, so either dodge out of the way of monsters or use firepower and well-timed attacks to take them down before James gets hurt.
  • Do not examine Mary's Photo/Letter, the Handkerchief, or Angela's Knife(after she gives it to you and James inspects it).
  • Choose 'Woman' for the Coin Puzzle in the Woodside Apartments.
  • Keep Maria safe. Refrain from breaking windows, bumping into her(yes, you read that right), and protecting her from enemies. She'll get grabbed by a Mannequin after opening the safe in Jack's Inn so I recommend making a save before you put in the code so you can make sure you save her before she takes damage.
  • Talk to Maria at certain times. Here are the times when you need to talk to her:
  1. Break the window into Room 108 and unlock the door. After leaving Room 107 with the safe code Maria will show a familiar outfit to James. Interact with her to discuss fashion.
  2. Walk up to Baldwin Mansion and interact with Maria to ask if she is alright.
  • You'll need to activate two different cutscenes during your stay at the Brookhaven Hospital.
  1. Go and check in on Maria after you pick up any of the three bracelets for the Bracelet Puzzle.

If the "Leave" ending is about James's devotion to Mary, the "Maria" ending is the opposite. Here, James leaves Silent Hill with Maria at the end of the game.
The requirements here seem to be the opposite of those in the Leave ending. Try to spend more time with Maria, keeping her close to James when they're together - so don't run away or abandon her. Help Maria first if she gets into trouble with monsters, and keep her from taking damage. Don't look at Mary's photo, and when Maria suggests something - like leading you to Laura when she's heading to Brookhaven Hospital - listen to her instead of heading off in a different direction to explore.

6) in the hospital you need to watch two cutscenes in her room. After we put the bracelets on her hand - we run to her to visit her.
The first time she will talk to you, the second time she will disappear.
7) in the labyrinth after each part we run into her room
8) in the hotel at the end we put a red apple in front of the mirror
9) after the battle with the pyramids, run into the door with the red egg
10) run through the corridor in the finale without listening to Mary
11) select the woman's coin at the end of the puzzle in the apartment complex.

People's versions differ, because you don't need to fulfill ALL of these conditions. However, you need to fulfill as many as needed for the ending to work... in my experience, it's about 80-90% of the points above.

Maria should sit on the bed as a result, then everything worked out. If she doesn't sit on the bed - then we boldly go to ng+..

I read that some people have a problem with this ending, I'll write how I got this ending.
Looking ahead, I was purposefully going for the ending "Maria" , but at the beginning they gave me "Farewell" , and then replaying the penultimate boss - "In the Water" and "Maria" .
- At the beginning of the game I looked at the photo and the letter once.
- Constantly kept 100% health throughout the game
- When I met Maria, I didn't let enemies near her, and if they attacked her, I reloaded the save (except for one storyline one)
- I always walked with her, and almost never switched to running, except for the plot episode.
- There were only two mini-scenes, the Dress and the House. I didn't go into the Historical Center, I didn't explore the locations, I followed the plot strictly.
- I often visited the Hospital, just stood nearby for about 5 minutes. There were two scenes, a conversation and a disappearance.
- I often visited the Labyrinth, there were no scenes, I just stood nearby for 5 minutes.
- I found a ripe apple and put it on a plate, I didn’t find a rotten one.
- I put the scarlet egg in the left door, the rusty one in the right. I entered through the left door.
- He ran down the corridor without listening to the dialogue.
After completing the game, I was given the "Farewell" ending.

- Next, without closing the game, I loaded a manual save before the penultimate boss, and lowered the battle level to Low.
- In total, I looked at the knife for about 10 minutes, about 5-7 times.

You can get 3 standard endings during your first playthrough. But I recommend going for this one, since it will be hard to trigger at the end, unlike the endings "Farewell" and "In the Water".
For this ending, you need to visit Maria more often at the points where she lies down to chill (Hospital) or sits in the plot (Labyrinth). You also need to constantly listen to her (open doors when she asks to let you in, go in the right direction when she says so, etc.), and also quickly fight off monsters. It is recommended not to look at Mary's photo in the inventory. In the corridor after the Pyramid Heads, do not listen to Mary's speech and immediately run to the door.
When you enter Mary's, she should be sitting on her bed. In the other two, she is standing by the window.

UPD (10/09/24): The guys who didn't get "Maria" for the first time encountered difficulties getting two endings at once. More questions than answers... I'll also add that breaking windows (Maria gets scared) did not affect getting this ending. No matter what anyone says.

The most important choice you make is the choice in the coin puzzle. When in the final stage of the puzzle, the top slot opens and you are asked who is to blame, choose the girl. This choice will bring so many points that if you visit Maria in the hospital 2 times (by watching the videos), you will practically not interrupt the ending, for example, you can calmly explore the city in front of the hospital, ignoring Maria's reproaches.
Be sure to watch all the cutscenes with Maria in Brookhaven Hospital, I had three of them, try to look into her room more often. Maria also doesn't like gunshots and glass breaking.

If anyone, like me, has serious problems with this ending, then please
Follow the basic and additional (lesser influence) conditions and you will get this ending.
Main:
1. Don't stay at critically low health levels for too long.
2. Do not look at Mary's photo and the letter (maybe 1 time does not count).
3. Don't let Maria take too much damage.
4. Try to do what she asks you to do, the main thing is to always follow Laura.
5. In BrookHaven Hospital, you must visit Maria several times and watch the cutscene.
6. In the labyrinth, visit Mary several times (the descent into her chamber will be to the right of the cube), each time you exit a new labyrinth, go to her.
7. At the LakeView Hotel in room 202, where you choose which apple to put on the saucer, be sure to choose the “Ripe Apple.”
8. At the end of the battle with the penultimate boss, you will receive two items - "Scarlet Egg" and "Rotten Egg"
You will need to go through the door into which you inserted the "Scarlet Egg".

I don't know how useful this information will be, but:
— when following almost all the recommendations for the Maria ending, I managed to get two endings in the first playthrough, doing exactly the same actions — Farewell and Maria. Absolutely all the actions were identical, except for one difference: the left and right doors with eggs after the pyramid heads.
— Scarlet Egg - right door, Rusty Egg - left door, went to the right - ending Farewell
- Scarlet Egg - left door, Rusty Egg - right door, went to the left - ending Maria

If you can't get Maria right away, just try to shove Aloe through another door.

Got three endings at once.
About Maria:
I examined the items exactly once.
He always broke the glass in cars where there were objects.
I didn’t listen to Maria where exactly I needed to go (I was constantly running, wandering, running away from her), I opened doors to let her in (where I needed to crawl)
I visited her in the hospital and there was a cutscene
I also visited her in the labyrinth
I never healed myself, I spent almost the entire game with red lives.
I used a rotten apple in front of the mirror
He put the scarlet egg in the right door, the rusty one in the left. He entered through the right door.

1. The coin puzzle does not affect the ending, verified.

The most disgusting achievement in the game (maybe except for the archivist). I still haven't figured out which trigger specifically affects what, it took me dozens of attempts to get "Maria". And everywhere they write different information.

The chronology is as follows (first playthrough, got "In the Water"/"Farewell":
- listened to Maria
- saw the scene with clothes
- I didn't see the cut scenes in the hospital
- visited in the labyrinth
- coin in the 1st puzzle - snake
- smashed glass, shone a flashlight in people's faces
- entered the scarlet door
- didn't listen to Mary's dialogue

This time it was impossible to get "Maria" in the end.

Went to NG+:
- coin: woman
- listened to everything that Maria said
- didn't shine a flashlight, didn't break glass (except for the required 2 in the motel and hospital), didn't push (he hit something once, but I don't think it had any effect)
- visited and saw 2 cut scenes
- visited in the labyrinth
- activated all additional dialogues (motel, near the Baldwin mansion and somewhere else)
- explored the city despite the reproaches (since he considered that they were leveled by everything else)
- Along the way I collected all the items for all the other endings
- walked through the door with a scarlet egg
- did not listen to Mary's reproaches

As a result - I opened 7/8 endings, but "Maria" was not given to me under any circumstances. I loaded to the boss fight with pyramids about 30 times, tried different approaches, loaded from the Lakeview Hotel - no way, "Maria" was strictly locked by something, I will express a hypothesis below.

Loaded an emergency save of the first meeting with Maria:
- as a base: coin - woman
- (since I screwed up the "Archivist" achievement) I only collected notes, the rest of the time I was with Maria (she was again indignant that I didn't go where she asked me to go, but only twice during our first meeting)
- he deliberately pointed the camera with the flashlight turned off and looked at Maria (he was walking with his back to her)
- in the motel during the first attack on Maria, he shot the monster (in the previous attack he stabbed him with a saw). Maria said "thank you, you saved my life"
- did not shine a flashlight, did not break glass (except for the required 2 in the motel and hospital), did not push
- visited and saw 2 cut scenes
- visited the labyrinth and stood for several minutes each time (i.e. essentially 3 times: the very first, after the 1st entry into the labyrinth, and after the 2nd)
- activated all additional dialogues (motel, near the Baldwin mansion and somewhere else)
- I did NOT collect items for all other endings
- in the striptease I looked at: 4 posters, a bottle of whiskey, a box with forgotten things, a nurse's costume, I went up to the pole so that Maria could ask "would I dance for her"
- walked through the door with a scarlet egg
- did not listen to Mary's reproaches

As a result, "Maria" was taken. I concluded for myself that for this ending it is very important (in addition to the main things, like the coin, the cut-scene in the hospital) to examine all the items in the stipukh, and NOT to collect items for alternative endings (looking at all sorts of scarves, photos, knives, etc. is also not worth it), otherwise I just can't understand what exactly before this, during the second try I locked this ending.
