I wanted to provide a visual guide for those that would prefer that. After you get the leg the monocle will lead you to a door that is now unlocked, and behind this door is the maze section you need to get through to find the jaw. 60 seconds is pretty lenient, even in the video I wasn't perfect and got lost a time or two but still got the achievement. I would recommend getting to the end where the jaw is, but not picking it up right away. The achievement will pop before you pick the jaw up, so sit there and make sure it pops first. Picking up the jaw will cause the game to save, and unless you made a backup back at the leg you won't be able to try again unless you play through the game to this point again.

Perform a manual save (USB or cloud save) after you get the doll's leg. A new rune circle will appear on the 3rd floor. Go to this door. You need to get through the maze in this room within 30 seconds. The timer will start as soon as you open the door.
Go:
-right
-left (a ghost boy runs across your path)
-right
-straight ahead (a ghost girl runs across your path)
-straight ahead
-right and quickly to the left
-slightly forward, then pass between nearby bookshelves through the broken section of wall around a table with a head on it, and there's a candle that lights right up
-right to follow the broken walls and through the shelves to a small pile of broken bricks on the floor
-left
-right
-right
-right to the small table with a jawbone on it.
If you did not get the trophy, reload your trophy Manual save.

You don’t actually need to make a manual save after you get to the door the achievement should have unlocked if not just quit out and retry the game doesn’t save until you pick up the leg so just keep trying until you get it

This is very missable. If you don't do it, you will have to start the game again and get to this point to try again.
There is a maze room that is unlocked after you get the leg. You have minute to get to the end where the jawbone is. The jawbone is the save point, but this should unlock before you get there - just don't pick it up if your achievement doesn't unlock. 60 seconds is a good amount of time, so you can afford to get lost a bit, but the basic route is fairly obvious. I suggest a few dry runs before finishing first before you go for it - your last save point will be where you got the leg so only a short walk to the start.
