This is best done while Hitler is facing forward or is on his side. There are a few decent opportunities to get this as he inspects the base. One of them is as soon as he enters the walkway where the U-boat is, the other is as soon as he's standing underneath the Ceremony Eagle. This will require timing and a bit of luck because sometimes the bullet can bounce in different directions. For a visual reference, see here:

This is the only kill achievement that isn’t environmental. At any point in the mission after you’ve identified Hitler as the target and while he’s walking around the facility, aim for his groin area and wait for the reticule to turn red. Fire your weapon and if you aimed in the correct area, you should get a testicle shot (the game is quite liberal with its hit detection). If you instead only wound Hitler and he falls to the ground, aim for his groin and shoot again. Once it is a confirmed "testicle shot," the achievement will unlock.
