For this Achievement you have to complete all secondary objectives in the main campaign. There is not difficulty requirement so I advise doing this at cadet or to your preference.
You can check your progress from the main menu by going to campaign and chapter select. If it has been completed you will see a green check mark. Some missions have so many that when you are in the mission you can't see them all. The easiest big picture view is from the main menu.

Just head to mission 7 , and go to the bridge leading to the graveyard there's a patrol boat thats roaming there and will appear as a secondary objective once you see it. You'll have to destroy it. Really weird that its not showing unless you discover the boat yourself, I think its the only secondary objective that you must do that in order to get the objective."

Secondary objectives/optional objectives can be given to you via the resistance members that you see at the start of the mission. Some will become visible in the main mission as well.
While going for this trophy, you will get the following trophies - Mother knows best
and Weaver's Warrior .For further information, see the respective chapter related trophies on what these secondary objectives are. Alternatively, you can find them in-game by pressing the
then 
They will show up on the left-hand side.This can either glitch or not glitch out on you. What causes it to glitch, I don't know. For physical owners, you can try and delete the patch to 1.00 and try a new fresh save. For digital owners, you will have to wait for a patch to fix this trophy whether one comes along or not, your guess is as good as mine.

At the start of every mission (except Mission 1 and 8), you begin in a safe zone populated by Resistance and Partisan members. Speaking to them will give you backgrounds to their characters, information about the mission and optional (secondary) objectives to complete. In some missions, there is also a radio where you can call ‘Mother Hen’ and get additional optional objectives to complete.
After you’ve spoken to everyone, you can begin the mission (it is not actually necessary to speak to anyone to start the mission [the objectives appear anyway]; it just adds some interactivity). If you press 
to view the map, it will show you all of the objectives available to you. Optional objectives will be marked by either a question mark or diamond in a circle and primary objectives are marked by a star in a circle. Pressing 
while looking at the map will list all your objectives (both primary and optional).
To unlock this achievement, you must complete all optional objectives in all 8 campaign missions. If you play a mission and only complete one or two optional objectives, you can select that mission again and anything you have completed will stay completed. It is recommended that you complete these on the Cadet or Marksman difficulty as doing them on anything higher might be troublesome.
Refer to the individual mission completion achievements for a list of all optional objectives.
Note: Even though there are optional objectives in both Overwatch missions and all the DLCs, completing them does not count towards this achievement.
