As you are progressing through the game it is important to check every chest that is appearing on the map to collect all the loot available in each region. You should also complete every side quest that shows up on the quest board. In addition you should complete all the background quests for each of your characters to get additional reputation points.
Do not hand in any artifact to the 4 representatives until you get to the end game. When you are on the quest "The Missing Piece" and the quest step is on "Report to the Council", this is the point where you should make a hard save.
Once you've got that hard save then pick one of the factions and hand in all the items that you can to that one faction, pop the relevant achievement, then reload the save and repeat for the other factions.
I started with Aristocrat, Lawkeeper, Lowlife, Sellsword backgrounds and after collecting all the items was able to pop all 4 of the achievements. Depending on the backgrounds you are running you may not have enough points for one of the factions. If that happens then I'd recommend working towards any that you missed out on in the second playthrough.
