NOTE: If you have already completed the game before it was patched on December 7, 2022, simply load your end game save file. As long as you met all of the requirements the achievement will pop.
For this achievement, you must create a New Adventure of the Main Campaign, Crown of the Magister. Custom campaigns and Lost Valley do not count. Difficulty doesn't matter, so pick whichever one you like - even Story mode counts. You must tick the Iron Man Mode box on the bottom of the screen. Your characters can't die a permanent death. As long as you resurrect them with a spell or Scroll of Revivify before the battle is complete, you shall be fine. The game will prompt you to do so if you forget and have the means to do so. The achievement will pop during the final cutscene of the game.

Also I'd recommend not choosing the spy background on any character, as that side quest requires you to be stealthy. Getting seen causes the quest to instantly fail, which counts as a death for some reason and will void your run!

Make sure you tick "Iron man" when you choose your characters before you start the game.
Late in my run, I discovered I could load two saves back by clicking the "Load" button in the dialog box in the top right. Only works on computer obviously, but if you sell the wrong item, you might be able to recover.
Outside of combat, there are a few ways to fail the game.
>Running out of food: Put a ranger or Druid in your party with Goodberry. (1st level spell) I prefer rangers, but druids are very versatile.
>Getting seen during the spy quest will fail the game. Do not choose the spy background during your run, or skip this quest if you do. If you rescued Daliat Sunbird, he offers to do the stealth segment for you if you go kill a garrison for him. Do that, return, and you'll get the achievement.
>Falling to death is only possible if you let the fly spell run out or be dispelled over a bottomless pit. Be careful and always end your turn over the ground when flying. Every ten feet is only 1d6, so it doesn't matter if you are high up.
>I had one error kill my character when she was pushed back into a tree trunk. I could have raised her from the dead, but instead, I was able to reload.
You could set the difficulty to the point where combat is trivial, but if you want a minor challenge, here are some party tips. Your party should have one main healer, one backup healer, one thieves' tools (open chests/disable traps) user, a Charisma-based character (should speak Orcish), and a detect magic/identify caster. I'll also include a goodberry caster. Being able to cast Fly and counterspell is nice, too.
>A balanced party would be a half-elf Oath of Timar Paladin (two-handed), wood elf Battle cleric (sword and sheild), wood elf Lowlife background marksman ranger (longbow), and a High elf court mage wizard. (cantrips)
