Very simple achievement, all you need to do is name your Chao for the first time. To reach the Chao Garden for the first time with a character, you must find the Chao key box in any stage, they're large blue boxes with an outline of a Chao's head on it. If you find one in one of Sonic's stages you will then be able to visit the Chao garden any time without a key in the level select mode. Same goes for every character on both the Hero and Dark sides.

Once you reach the Chao Gardens, visit the only open garden (Neutral), you will find two eggs, hatch one with one of your characters, eggs can be hatched by simply waiting, or sped up by picking them up with the B button, and while still holding B move around the directional stick to massage it and then after about 10-15 seconds doing this it will hatch when put down. Or alternatively if you want the quickest and meanest option you can simply pick up and throw the egg for instant hatching. (Your Chao won't appreciate this)

Once hatched, pick up your Chao with the B button and exit the garden with him, you will now be in the lobby area, directly opposite the Neutral garden is another doorway, go through here. You will then come to a room with doors to many different areas pertaining to your Chao in different ways, the first door on the far right will be the fortune tellers, go to this door with your Chao and press B.

Here you can name your Chao, the fortune teller will suggest a name for you, but you can turn her down and choose your own name. And that's it.

Chao! - 20G (Original Achievement)

This might seem a bit complicated but its actually easy to do.

Simply play any level you want and find a Chao Key, then complete the level.

For example, play City Escape as Sonic and find one of the large blue Chao Boxes. Break it open and you'll find the Chao Key, grab it. You'll notice it appear next to your ring counter, from here simply finish the stage to gain access to the Chao Garden.

Now we dont have full access yet so immediately leave through the cave and head back to the story. From here back out to the main menu and go to Stage Select. Near the North West or upper left of the map you will see a picture of a Chao, this is the Chao Garden. Seeing as Sonic got a key, he is allowed all access to the Garden and Kindergarten at will, so select the Garden and Sonic and load it up.

Once inside go straight forward and you'll be inside the Garden you were once in. Stand next to one of the 2 eggs and press and hold B to pick it up, while holding B hold the control stick left or right to "hug" the egg, then after maybe 10 seconds, let go and press B again to put the egg down. It will hatch and a Chao will come out. Press B to pick it up and leave through the cave you came in back to the Lobby.

From here run around to the other side to get to the Chao Kindergarten and you will see 5 doors and a locker. You want to pick the 5th door (the purple one) which is the Fortune Teller. Once she sees your chao she will give it a name and ask if its okay, say no and she'll offer to try again, once again say no, now the 3rd time she will ask if you'd like to personally name it, say yes and pick whatever you like. There you have it!

After you enter the Chao garden Via the Stage Select map pick up your chao and walk out the door. On the opposite end of the lobby will be another portal. Once in the Kindergarten, walk to the Purple door and the fortune teller will ask you to name the chao. Type in whatever you want then bam, Achievement Unlocked.
