To unlock all combat upgrades you have to complete 160 challenges in the Web of Destiny and then *buy* all combat upgrades (it didn't pop for me until I purchased every last one).
Each level (except the tutorial & Mysterio) has 15 challenges, most are pretty straightforward, but if in doubt consult the guides for the Manifest Destiny achievement (completing all challenges).

Buy all of the combat upgrades, pretty straight forward. You may have to play levels over again to get enough Spider Essence.
You can view what you have and what you need to buy by pressing 
or 
and going to the Combo Upgrades.
