Splasher

Fairly simple, all you need to do is die by being crushed by one of the doors. Hit the switch for a door so that it's open most of the way, but not locked open, then stand under the door. When the door falls and you die, you will the achievement will pop.

I got this midway through level 10, but there are numerous easy locations throughout the game.
