A simple way to get this achievement:
Play the game on "Savage" difficulty setting, get every photo of Jen and listen to all of the Doc's recordings. As the credits roll, you should get the achievement for beating the game, beating it on "Savage" and unlocking 50% of the journal.

You also need to unlock 35 skills (or more) or you won't reach 50% by the end of the game on savage.
With every collectibles I was only at 49% and feared the game glitched on me but right after starting my new game+ on brutal the achievement popped on the next skill I purchased.
