Each Mission (save for a few missions) contains at least one Secret, which is a stash of ammo and/or inventory items that usually take a few tricks to reach or unlock. In total, there are 34 Secrets to find.

Some have claimed that this trophy has glitched on them, but it most likely is a result of three factors:

1. they actually missed one but forgot which,
2. they used cheats and forgot to take them off when getting one of the Secrets, or
3. they accidentally deleted the Secrets Save file.

In most cases, it seems to be the latter. Since you cannot overwrite saves, you eventually have to free up space by deleting some saves. You MUST be careful not to delete the Secrets Save file, otherwise you will need to do an entire playthrough all over again.

For this reason, I don’t mess with cheats until I have successfully gotten this trophy in an entire playthrough.
