The entrance to the Phon'qi Caverns is in "Foothill Falls" in Koboh very close to the Cantina. The entrance is in a shack near where you meet Skoova the fisherman.
It will probably take you about 25-30 minutes to go through and explore the caverns but for this achievement you need to make your way to the end of the cave and will find an echo where you learn an important secret about a certain character.
Shortly after that echo is the exit of the cave and Cal & BD-1 will have a quick conversation about what they just learned and the achievement will unlock after that.
Make sure to grab the legendary enemy, the fish collectible, the bounty hunters, and plenty of the other collectbiles here if you are going for 100% completion.

I've just completed this area and got all the area collectibles, what was this character secret we learnt. Did I miss something?

Just finished Chapter 2 at the Forest Array and got the rumor from a prospector in town to visit the shack. Can access the caverns underneath and recruit the droid there for the cantina but the point at the meditation spot in the video is actually blocked by an object that requires Force Lift I believe in order to access the rest of the caverns. So...this achievement is locked behind a power progression from chapter 3 I think?

The Phon'Qi Caverns is accessible from the mysterious shack in Foothill Falls, and is part of the "Investigate the Abandoned Shack" rumour that you can get from a Prospector in Rambler's Reach. You'll want to tackle the Phon'Qi Caverns with all of your abilities, so come here once you've finished the main campaign.
It's a good old fashioned dungeon, this, and it comes with an excellent reward at the end. In the final room, there's a piece of machinery to interact with. Do so, and you'll unlock this trophy, along with a Map Upgrade that gives you the ability to see any unfound Chests, Stim Upgrades, and BD-1 Upgrades. Very nice.
