This is a work in progress but a there are a couple points to note before progressing far into the main story.
*Main Mission 10 NO SUDDEN MOVES is a point of no return of sorts. In the following mission,
Companion affinity is increased via dialog options with them or in front of them that the favor, making certain decisions in quests and time spent with them in your party. Companion affinity is decreased by saying things and performing actions the respective party finds unfavorable. You will be notified of the character's feeling towards your action by a prompt in the top right corner of the screen stating their feelings.
*Leadership Perk Rank 1 increases companion affinity gain by 25% (although this seems to be unnecessary if you're just looking to get this achievement out of the way).
*Each Companion has a personal quest that will delve into their backstory. This must be completed before any progress is made on their affinity meter. Additionally, they will often request for you to have a conversation with them, furthering their backstory. Ensure you have these conversations as they pop up.
*Romance is of course an option. If that's your prerogative with your companion, then choose "flirt" at every opportunity when speaking to them.
The following is a guide to each character capable of a Maximum relationship level. I'll be updating as the community figures out exactly how to act. Don't worry about this one too much. It's a massive game with ample opportunities to maximize multiple relationships.

You dont need to do the main mission at all to unlock this. I progressed only to where you get the 3 missions for the other different companions and then went about doing the UC/Vanguard faction quest line. Still not even done with the whole UC quest line and I just had the ceremony with Sarah and the achievement popped.

To give some idea of how long this takes, this is what I completed, started off fresh from a new game+ with Sarah:
I had the perk Leadership - rank 1 perk for companions to gain affinity 25% faster.

It might be an idea to buy/make as many Paramour (Hippolyta / Synapse Alpha / Hypercatalyst, ) and use them every time you are in a major conversation, might boost things along quicker.

Bethesda Game Studios has made a concerted effort to improve its core relationships in Starfield, and that is very much apparent. In all, Starfield has 4 romanceable/super friend options (Sarah, Sam, Barrett and Andreja), each of which actually has its own Mass Effect-like loyalty mission at the end (although nowhere near that kind of quality).
Basically, to get a companion to the maximum relationship level, you need to take them everywhere with you, and make dialogue choices while in the game that they would approve of. In other words, when you’re with Sarah, don’t be a dick, don’t be evil, don’t steal, don’t kill innocents, and when you get a chance to speak, talk about how you love Constellation and exploring the galaxy. Just generally be as nice as possible.
Then, at various points throughout the game, the companion in question, whoever that may be, will ask to speak to you, and they’ll reveal more about themselves and open up more. This process then goes on and on: you do things that they like, and they’ll eventually ask to speak to you as you climb the various invisible tiers on the relationship tree - it'll even create an entry in your Activities list too (which will help with Fixer (30G)).

- If you want a romantic relationship, when you get the chance choose “Flirt” as the conversation option.
- Try to stick to one follower companion until you've unlocked this achievement. Once you’ve done one, then you can share the wealth and try the others.
- Different companions give you different gifts every so often - Sarah gives you organic resources, Sam gives you inorganic resources, and Andreja straight up gives you credits.
- Responses to things you do in the world range from this character “Dislikes that” and”Hates that” to “Likes that” and “Loves that.”
- If you’re going to do something dodgy, leave your companion on the ship, or ask them to wait around the corner while you go and do something shady.
