How to unlock the The Stars My Destination achievement in Starfield - Definitive Guide
The key to this achievement is a high level grav drive, upgrading your Astrodynamics skill, or both. I would highly recommend both! Patience is a must as well as there are 120 Star Systems. Thanks for counting, Xero Deathz!
If you want to maximize your chance for this early in your playthrough, I would recommend just using your grav drive to hop between systems.
If you want to do this and earn some XP/Credits on the way...
I can list out each Star System you're looking to go through but there's really not much to this entire process. Be cautious that some Star Systems contain more than one Star. So they'll sometimes be stacked upon each other.
I spent some time to count things out and have a list of what I think are the 20 Binary Systems in the base game (not including NG+) as well as the only Three-Star System. From my count, that leaves 77 individual Star Systems. I do not plan to list those out but I can with pictures if people request it in the comments. If I've made any errors, let me know and I'll happily update things.

The way you track if you've visited the System before is it will be light up brightly if you have been there before and it be faded if you have not. Another method is to highlight the name. If you haven't visited it, it will only display the Name and Level. If you have visited it, it will show more details below the name. Thanks, lucylu1983!
In a more official way to count, as mentioned in the comments, you can press , then , then scroll down to Exploration. The top of this section tells you how many Systems you've visited. Thanks, Ssj Sean!
I cannot confirm how NG+ works, but I assume you just need to visit 120, even though the achievement description says all. This is based on feedback in the comments.

Should add to the guide that there are 120 total systems to shoot for. Took me 3 hours of grinding

I can confirm, the name of this achievement is wrong, it should be ´´Visit 120 solar systems`` because if you visit 100, just start NG+ and visit another 20 solar systems and the achievement will unlock normally.

Just had this pop myself. The other guide is pretty perfect, just get a ship with the best grav drive and put all your points into it and then bounce to every star system you can. However I would like to add that I do think that Unity counts, atleast it did for me. If you're far enough to understand what Unity means then right now you're probably going "ahhh" otherwise I'll just say finish the game. I was stuck on 119 and couldn't get it to pop, I finished the game and it popped during the credits so that's my only real explanation as to why.

Then there must be 121 systems, because I got it without ever going to whatever Unity is.

This one just popped for me. The easiest way to do this imho is to do a playthrough of the main quest, go through the unity. The ship you get in New Game Plus is more than sufficient to complete this quest. This way you don't have to learn shipbuilding or modification. I had Astrodynamics level one as well. Not sure if it was necessary.
My stats (press start and then press Y, and look under "Exploration") currently show that I've been to 148 systems. Not sure if it popped at 147 or 148 because I didn't see the notification.
Be sure to get Van Maanen's star. It's very isolated off to the left in the Galaxy map, and is very small and dim. It blends into the background. Beyond that be sure to get all of the binary star systems.

This achievement unlocks when you have visited 120 star systems. Some tips to do this:

3. You can jump to the system and then immediately jump away, and it still counts. No planet landings or space dogfights required. Just survive and get away.
4. There are actually 121 systems that you can encounter in a playthrough. One appears in the main story that you otherwise cannot find, but there are 120 statically available.
5. Visited star systems will have a glow around them versus star systems you haven't visited.
6. Star systems that are outside your current location's reach and never visited will be red. Find the closest system mid-path between the target system and jump there first. If this is too frequent, see tips #1 and #2.

Can confirm you can visit all Star Systems in one playthrough.

The achievement should be 120 different systems.
I recommend doing it in NG+ because the count keeps growing but the systems you visited are reset and recount. It's much easier than looking in the stars for the systems you've left behind...

Visit all 120 systems to unlock.
Proper map to see where every star system is. If it doesn't show survey when you hover over it that means you haven't visited it.
If you have 119. You must finish the game to unlock the last.

For some reason this achievement unlocked extremely early for me. I had about 30-40 systems left to visit on my travels and had it unlock prematurely. I wonder if there is something that causes this.

There are 120 different star systems in Starfield, housing 1,000 different planets and moons. In order to get this achievement and Stellar Cartography (10G) you’re going to need to visit every star system.
Now, this might sound daunting, but the truth is, it’s not at all. For one, it doesn’t take you long to see the whole galaxy in Starfield’s starmap (just zoom out with and you can see most of them with a 3 or 4-second scroll); and secondly, there’s actually a colour code system to tell you where you’ve been and where you haven’t. Let’s run you through it.
