After you reach any ending on Day 16, you will be able to continue the game on Day 17. You will receive a note on your desk about the Monastery of the Black Veil. Solving this (see below) and visiting the monastery will provide you with all the pages missing from your plant guide. Ring the bell and Simone will come by and ask you to identify all your remaining plants. Do so, and the achievement will pop.
If you killed Burbidge earlier in the game a police officer will arrive to arrest you. I believe you'll have to poison him as well if you want to complete this achievement.
To visit the Monastery of the Black Veil, first go to Ravenglass (J6). Make the Elixir of the Black Veil (storium, thum, and twilight lepiota). Visit the monastery at I12.

I can't get this to unlock I've done exactly what you state here went to Ravenglass made the elixir and then went to monastery. I got the pages but the achievement has not unlocked. What am I missing do I have to do something else?
