If anyone is wondering, ghosts DO NOT count, even if you spawn them by breaking their gravestones. Surely then, it works even if they were spawned and walking around from the start of the floor without having broken their grave stones, but idk for sure.

I randomly unlocked this while attempting (and failing) to join a friend's online game. Not sure if this can be replicated or if I just got lucky, but I believe I joined at the same time that the host ended the run. I was the only one that unlocked the achievement.

Pretty easy. Before starting a level (after completing the tutorial), talk to the Mutator character. Turn on the mutator for Infinite Ammo, and perhaps No Cowards so you don't have to chase guys after you start shooting them. Start a level with the Soldier, probably an easy level like Slums. Switch to Machine Gun. Go around shooting everybody. The achievement pops before exiting the level, once you kill the last NPC. If it doesn't pop, you probably have to search the level to find a straggler. If you search and can't find somebody, just go to the next level and try again, it's generally pretty easy to do, so if

Infinite Ammo is definitely helpful so you don't run out of ammo while trying to kill everyone, but you should also turn on No Cowards so that people don't run away from you when you open fire, which makes it a pain to track down stragglers. Play as the Soldier so you start with a machine gun, making it way easier to gun people down. This achievement will unlock immediately in the middle of a level once you've killed everyone, so if it doesn't unlock it means someone ran away and it hiding somewhere, such as near the entrance or exit of the level. This is easiest done on the first level of Slums because it's the smallest and has the least number of people on the map to kill.
