Streets of Rogue

As the description says, you need to have four status effects affecting you simultaneously. These are displayed in purple on the upper left portion of the screen, under your health.

Status effects in this game are quite different than what you'd expect: there is no status effect for being poisoned, or on fire, or whatever. It's actually way easier to get this achievement that you'd imagine, because here are a variety of things that give you status effects:
 

• Bribing a cop gives you the 'Above the Law' status effect for that floor
• Drinking a Resurrection Shampoo gives you the 'Resurrection' status effect until you die
• Syringes that you find will always give you a status effect, and each one is independent. These are the ones I'm aware of:
  • Slow
  • Weak
  • Invincible
  • Health regen
• Equipping armor counts as a status effect. For example:
  • Hard Hat gives you some melee resistence
  • Bulletproof vest gives you bullet resistance
  • Armor that buffs your damage does the same
• Getting bit by a Vampire gives you 'Dizzy' status effect

Those are just some examples, and that's ten right there. You can quickly see how many options there are to get four at once. The most common thing for me was finding different types of syringes. With the 'Drug-a-lug' trait, you can see what they are too. Once you have three different syringes, you can just bribe a cop on that floor and use the three syringes to unlock this.

Alternatively, to eliminate any RNG, you can turn on the mutators for Longer Status Effects and Random Status Effects. Start up a run, and you'll be getting random status effects applied to you over and over on a cycle. Longer Status Effects will eventually make them last long enough that you'll get four at the same time. This still requires a bit of RNG to get ones in a row that last long enough, but you can just idle for a few minutes until this unlocks if you can't do it normally with the above tips.
