Surgeon Simulator

This trophy follows right along with the description. Simply get the eyes to pop out using your preferred method (ex. stab to the eye with a sharp object, followed by vigorous shaking of the instrument), and then cut them out using the axe.

This trophy did glitch for me the first time, but unlocked on the second attempt. For easiest results go to the eye surgery level in the operating room.
