You can find the Flask of Homunculus besides the Inventory chest, at the right side of the Altar below the stairs. The flask won't be there after the tutorial, you must play the dungeon floor first. Have the flask on an active slot (

) and it will be automatically used once you enter the dungeon. Now here is the thing: just using the flask on normal conditions will have no effect and no achievement, you must have a second controller on as you enter the dungeon and the flask will create a copy of you. Achievement unlocked!
