After you defeat the Necromancer for the first time, the left door by the Altar will be open again. That's the same door you went through at the beginning of the game, for the tutorial and acquiring the Sword of the Necromancer. There you will find 3 Slaaims monsters and a sword on a table. Just pick the sword and you will get the achievement.
