I will try to keep this as spoiler-free as possible.
You will fight the Necromancer two times. For the second time, you must use Tama's Sword when you defeat the possessed minion. You can get Tama's Sword by going to the left door next to the Altar.

If you use Tama's Sword, you will get to fight together against the Necromancer (and a prompt to do this co-op if you want to). I call this the "good ending". That will give you the achievement.
If you forgot to swap swords and used the Sword of the Necromancer instead, you will get the "bad ending". After that, playing as Tama again, just interact with the hourglass by the Altar to go back in time and go through the dungeon floors again. Just don't forget to get Tama's Sword this time!
