In Practice Mode be sure to select Tag, choose two characters, and once it begins...

Go up next to the opponent and press X+Y+LB.
If entered correctly, you will flash and your partner will come out with you. Feel free to add a couple quick jabs or kicks to round out a combo.

Do this ten times.

A tag assault is when you use a bound move and hit the Tag (5) button immediately afterward, which will bring in your partner character to attack, while your main character stays on the screen to attack directly afterward. This is taught in Fight Lab.

Example:
Law, Kazuya: /+, ++~ Tag Assault! Kazuya ,, Law ,+, (d/f+2, f+1+2~5 Tag Assault! Kazuya 1,2, Law f,f+1,2)
Jack-6, Steve: /+, ,~ Tag Assault! Steve +,, Jack-6 ++ (d/f+2, 1,1~5 Tag Assault! Steve f+3,1, Jack-6 f+1+2)

To do a Tag Assault you need to press the tag button right before hitting the opponent with a bound attack. This will cause your partner to come to your side for a brief period with a white outline around them. During this period the both of you can attack the opponent until your partner goes back. Repeat this ten times for this to unlock.

NOTE: This can be done in Practice Mode.

Same as the "True Master" achievement. Approaching the opponent, hold down LB, then actively poke the X+Y combination until the achievement appears. I did it in training mode.
