Easily obtained if you get the "Completion Scroll" in this dungeon. This will show a small mark on each This might be a little tricky to get.
One of the lower left rooms has a contraption that lets you move a mobile spike shooter up and down. Use this to activate the button on the wall and also shoot a spike through the hole in the wall. This will lower 2 walls and in this new area is the scroll which will make it tremendously easier to track the status of each room as you pass it.

Game bugged. If you can’t unlock achievements but you finished total, just start last level again and achievements will be popped.

Collect the completion scroll and then complete the map with a check mark on the pause menu. Completion is found from starting position by going right, and then down. Use the button in the lower right corner to move the thing up and down that is shooting an arrow.
You need to active the clearly visible button to move on in this room, but there is also a small slit that you can shoot an arrow through. This opens a hidden wall on the left which leads to the area with the scroll.
Note that to complete rooms with hidden doors that lead to another area with a collectible, you need to pick them up too.
