The Scroll of Completion causes a check mark to be shown on each map room that you've completed. It can be found above the top room of the second column on the map on the Chalice of the Gods dungeon.
Make sure you have two bullets before you enter the room. Once you're in this room, climb to the platform on the top left as you see here.
Shoot the first two cacti, then jump over to the platform they're on. Jump on top of the third cactus, then jump up through the gap above it. You'll now be in the room with the scroll. Touch the scroll and the achievement will unlock.
