This is preety easy without even boosting:
First, you have to survive a direct Scar hit. Just play a level with the Scar gun in any mode, let anyone to hit you with the Scar, and then run away. Probably people will want to hurt you even if they don´t kill you, so you´ll get it easily.
For surviving 500 of damage, try to play a Deathmatch (Team is better, because it´s harder to die in a moment) on the small square level where, if you play Butcher Bay mode, all Powercells are in the middle of the map. If you go through the small tunnel on either side, you will get a heavy armor (+100 armor) and heavy health (+100 health, up to 200). Then just keep playing killing people, and get armor and health as much as you can. You will get it easily too.
Lastly, for surviving an entire match, just play Pitch Black on any level, and if you don´t start as Riddick, just don´t fall down and wait for the game to end. Better is there are at least 3 people so you don´t annoy anyone, and 5 minutes.
After getting these three things, you will unlock the achievement. You can view your progress in the main menu of the game.

In case anyone is wondering, you can find the Scar Gun on Desolation Court. It's on the upper level near the 'far' edge of the map.

Scar fires sticky grenades. Shoot with RT, then detonate with LT.

Survive a direct SCAR gun hit:
The SCAR is a grenade launcher that fires blue orb looking grenades that are remote detonated. Not to be confused with the standard grenade launcher. You can find one on the "Desolation Court" map, it's at the far back on the upper level. (Opposite the Minigun and past the "Damage Boost") get your boosting partner to shoot a single orb onto you then have them press
to detonate it, its important you have full health or the blast could kill you! -
Survive 500 damage:
For this you must get your boosting partner to shoot you a few times then heal up. You heal by picking up the red vials that are found on the ground around the map. A good way to do this is use the grenade launcher, get your friend to fire a single grenade at you that will take your health down to 5-20 then grab the vials until you reach 100 health and repeat until you have gained more that 500 health back. -
Survive a whole match and win:
This is very easy, just play a Deathmatch and kill your boosting partner over and over until the game ends. You will get this while boosting kills/wins to.
