The description for this trophy is a bit misleading, it just requires you to visit every named location and city in Marca Hispanica. Each time you make a run through the area just concentrate on going a different path and hitting all the named locations and this trophy should pop on your 6th or 7th run. Keep an eye on the map layout at the start as different landmarks will appear randomly.
I'm not sure if some of the quest locations given in the earlier location count, but just keep those in mind if the trophy won't unlock.
