The idea here is to have Sean light 100 cigarettes. There are 2 ways to do this: stand still and push LS in or just stand still for a while. Obviously, it's much faster just to hit LS and tell him to do it rather than waiting for him to light one on his own. You must be outside before you can light up.
If you wish to light several cigarettes in a row, when Sean is holding the currently lit one in his hand, move and he'll throw it to the ground. If it is in his mouth, he'll walk along with it still there, blocking you from lighting another one.

This will get you the achievement (or at least a good amount of progress towards it) by the time you are done with the story. Worth mentioning that it shows how many cigarettes you've smoked on the stats screen. Second page (the one actually titled Stats, not Progress) at the very bottom of the list.

I was able to complete this in about 25minutes by doing the following:
1. Click the L stick to initiate the smoking.
2. Once the lighter is back in his coat move slightly in order to drop the cigarette.
3. Repeat.
Note:This will get boring so do it while your busy doing something else like talking on the phone etc.

I found the easiest way to obtain this is to plug in your controller or use a wired controller, go into an open area in the game, and jack your right analog stick to the side.
Walk away from your xbox or go to sleep and in a little while you will have smoked 100 cigarettes without idling out or pausing.
The screen will spin but you will still smoke til you get cancer, and the achievement of course

Used the plug and play controller, had Sean hang out in the slaughterhouse HQ, walked away for a couple of hours and the cheeve popped.

When standing still click in both joysticks and Sean will give himself cancer. Do that 100 times in total and you'll get this trophy. He'll also do it by himself if left alone so you can just watch tv for an hour or so and he'll take care of all the work.
