One can be gotten after you find the underwater cave in the cemetary. You must first dive and get thru the underwater portion to enter the open cave where you will have a conversation with usha.
After you finish talking to Usha - the easiest way is to not tell her she must die. The fight will be long.
you can just say ‘Bye’ and shoot her in the back of the head which will get you two achievements at the same time.
