This can only be unlocked under the Challenge Mode menu, as that's where you can set the difficulty to Hard whereas Adventure Mode has no difficulty setting.

This is very easy on Hard however, as all you need to do is keep a close eye on the Skull Card and make sure you don't pick it each round when the fortune teller is done shuffling the cards around.
