Realistically this probably will happen on accident, but here’s how to do it!
Chase a victim and get them near an exit gate, ideally want to lower their health first, then kill them. Make sure you get the final hit! Would advise high Savagery for more damage. Everyone but Sissy has good Savagery

the way i did this was as leatherface down a survivor then carry them near an exit then drop them and kill them if you're close enough to an exit it should unlock

The exit DOES NOT have to be opened for this to count. I got this against a victim that was next to the closed pressure gates on the Slaughterhouse map. Not sure the exact range that counts but it seems to be a wide area around and at the exit.
