On this difficulty, the player receives a -40% penalty to all elemental, poison, piercing, bleed, and health resistances. Otherwise, the difficulty has not changed much from normal - you will meet the same enemies, whose health will be noticeably increased, as well as the damage dealt to you. At this difficulty, legendary items (purple) will begin to drop for the first time, and you will also be able to find 3 optional ultra-bosses (Talos, Manticore and Dracolich) that will thoroughly test your character.
