The Swan Diver I achievement is acquired on the Mediterranean Sea level. Immediately after killing the Kraken, face the wall and move to your right. You should find a nook and wall kick until you reach the upper ledge. If you did not already have it you should have acquired the Master Climber achievement.
The Master Climber achievement in Tomb Raider: Underworld worth 14 pointsExecute a chimney jump of at least 4 wall kicks
There will be a treasure on this ledge and to get the first Swan Diver achievement, jump off this ledge and into the water below. To swan dive jump (A) and press (B) immediately.

***NOTE: To get the Master Swan Diver achievement you must complete Swan Diver I and Swan Diver II in the same playthrough of the game. Luckily, all of the dives are in the first half of the game.

think i found a leak in the solution...there is no relic... but is a treasure otherwise the relic achievement would be up their as well... you can also do this on the other side of the room as its the same height.

A guide for anyone who is just after this Dive or has missed it previously.
For this Achievement you need to be on the Mediterranean Sea level.
It'll be the area after killing the Kraken when you pull the lever. From here turn to the left or right to see a groove, now wall jump to the platform above. From this platform I found it best to do a bit of a run and near the edge, press 'A' to jump then quickly press 'B' to dive into the water below.
Please note you must do all dives on the same save to unlock the master swan diver achievement.
The video shows the main area in case you have already killed the kraken and are trying to get your bearings.
