To make a potion, you simply have to mix two ingredients together. Soon after you reach mainland in the storyline you'll be required to do a quest for a lady to help her horse (south exit of the town), at the end of this quest she'll teach you alchemy and this is your first opportunity to create some useful potions.

Early on in the game you will learn how to create potions, and you will unlock this trophy at that point.
To create a potion, first enter your inventory and scroll across to the Alchemy tab. Now press 
on at least two different items here to add them to your cauldron. Once they are in your cauldron, simply press 
to brew the potion. The more ingredients you add to the cauldron, the more powerful your potion will be.
