Multiplier is where you score much higher points. If you get hit or did not kill any enemy as fast as possible, you'll lose a multiplier by 1.
The best way to hit 20x is level 3 (in single player). While you're at it, try to avoid getting hit by demons' fireball or melee attacks. When you're facing off a fat & tall demon, move around the circle (to avoid his ground fist) and shoot at him until he's dead; if you want to kill him fast, use your special attack near him (for me, I use Mark Lili's special in order to stun him and deal more damage at the same time). Also, watch out for Succubis, as they shoot out fireballs. You should hit 20x near the end of level 3 (if you did not get hurt too much).
