Every time you bring the vaccine back to your friend you go up a rank and get an achievement. The house will shift and randomize, you'll earn extra time and you have to go find the vaccine again. (If you solve the mystery of the house the game will end and you'll have to start a new game so ignore the light beam puzzles and notes when going for the rank achievements.) To get all the rank achievements you'll need to be fast so just get the vaccine and bring it back to your friend but try to collect as much as you can so you can survive. The first time going through is the hardest since you only have the knife to start out with. You'll need to find a pistol and/or shotgun and some ammo so you can kill the tyrant by the vaccine and get the custom pistol. After that it will be downhill (as long as you have ammo) you just have to hope you can find the vaccine fast enough with each rank and don't run out of time. There's a lot of luck involved since the house is sometimes bigger and harder to find the vaccine or you might run into a lot of crawlers without having a gun or repellants or you might not find much supplies/ammo or a firearm.
You also get XP and can level up your stats. You want to level of your aiming since it will make your guns do more damage and also your health since you'll take less damage. Also leveling up your luck helps you find more supplies.

thanks for the visual assistance. I just had my first successful run and couldn't find a gold key in time for the undead achievement. Gonna give it one more go and complete the story this time and run up to undead again. Biggest tip I got: doors open inward. To find your way back, make sure you are always going with the door. It will help you find your way back if you end up in a long twisty maze to find the vaccine. Almost didn't make shark but that one saved me.
I used a slightly different strategy and have to admit it takes a bit of luck. I started by finding a repulse potion or w/e its called and then stabbed a crawler to death, while on said potion. You get boat loads of xp (like 77 which is a couple levels) which I put in to hp and aim. I did this 3 times in the first level before I started using my pistol on crawlers but still killing the zombies with a knife. I think I was lvl 6 and 7 for both and 3 on luck by the end of the first round. I killed EVERY zombie/crawler I came across, the whole game.
I opened and shut all doors two times for 1 xp both times on my first two runs for the bonus xp. by my 3rd run I was nearly maxed out. By my last round I had 105 pistol rounds and 60 shot gun shells 6 large med packs 9 repulse potions and some useless prox mines I didnt want to discard in case i triggered them. For the remainder of the game I used the pistol on everything accept the big guy at the end. I shot gunned him while standing in the door way.
Also it is advised to run everywhere in the game, in the WT. I suggest walking when the camera is close to changing angles because at first I was running in to a walker hug from hell.
All in all, the game could be better but it is fun once you get the custom pistol. That first level though... damn.

You receive the Rat rank after completing the first Vaccine run, with or without completing any puzzles. You have to do this in 30 minutes or less. You can use the gun if you would like, but you should be able to kill most of the enemies you come across with just the knife. The trophy will unlock once you return the Vaccine to your partner.
If you need a reference, Spider and Mystery Solved both have videos showing this rank being earned. Just note that the houses are randomly generated, so your house will be different than that shown.
