In this video guide I will show you how to complete a mission without killing civilians. This can be done on the second level. Later in the level there will be two scientists, make sure they live. Do the fighting on the lower level to ensure they live. When the fight is over go check on them, they should both be on the ground cowering and shaking. That is how you know they are alive. If they are alive finish the level and the achievement will pop.

It is a bit bugged in that, if you happen to kill a civilian, you cannot get the achievement even if you don't kill a civilian for the rest of the game. That happened to me. What fixes it is deleting your whole save off your Xbox and starting fresh. If you miss this achievement, wait til you get the rest, wipe your save, and then try again. It popped for me that way.
