To win the United States Championship, you need to be rank 3 of the United States division and locked into a feud with the champion. The championship match should take place at the pay per view.
NOTE: You can begin a feud with the champions by doing a call out promo, going to the champions tab and selecting the current champion for your division. If you do this when you are rank 4 and win your match for the night you will move up to rank 3 and automatically qualify for a title shot.

Not sure if it got patched but the call out at 4 worked on the NXT and Tag belts but did not work for this the IC or World title it put me in a rival match with the champ but only for 1 match and even though i won the match it said i lost the rivalry so i had to pick the #1 contender to feud with just to move up to 3.

Yeah, the game has major glitches, especially in career mode with the feuds. Career mode was so unbelievably frustrating with the glitches I quit playing the game three times and it took me three months IRL (kept having to wait to see if patches would help) to finish getting the titles, and by then I had everything purchased (and Heyman).
Anyway, for the World title I didn't get put in a feud in January (Well I did but then I left the World Title division and went back), won the Royal Rumble, that then I was granted a World title shot at Wrestlemania. I've heard that can glitch as well. There was no feud with the champion, by the way, which is dumb as hell. If it does glitch, you can win Money in the Bank if you're not in a feud. I had to go that route in 2k16 since I didn't get my title shot at Wrestlemania after winning Royal Rumble.

Earning the titles themselves is pretty straight forward. Join the ladder for that title, do some rivalries with people in ranking, cut promos, etc etc. Basically just stuff to increase your popularity.
However, here is a tip on how to get the United States Championship easier. If you are in a different ladder (for instance, going after the WHC) and switch into the US ladder (or any other ladder for that matter), you will start off at rank 10 in that ladder, meaning you have to climb from 10 to 2 in order to get a title shot.
A way to make this quicker is to start a new MyCareer in a different save slot. Depending on how you do in the training, you can start in 4 different spots, NXT tag team, NXT singles, WWE tag team, or WWE singles. When you are in the training part, make sure you beat Jayden and Samoa Joe in the matches and beat Enzo in the promo battle. I have no idea about the self-promo, but just try to fill up the bar around at least half way to be safe. If you do all of that, you will end up on the main roster in the US championship ladder at number 5, which makes it a lot less of a climb than from number 10.
