How to unlock the Title shot achievement in WWE 2K17 - Definitive Guide

Select a 1 on 1 match on the main menu. I selected a Tables match, but a Normal match will suffice as well.
Now select two characters and after you pick the 2nd player press Y for Match Options.
Most of the belts are locked. Scroll to one of the available belts. It doesn't matter who it's assigned to. It will still be defended.
Once you win the match and win the belt you will see on the top left side of the screen say "Break" and press the A button to allow you to continue the fight after the match.
Approach your opponent with your newly one title and hit him with it. It may fall out of your hands as soon as you press the A button to continue to fight, but just simply pick it back up with the Left bumper.
Achievement will unlock a couple seconds later.

"newly one" should be "newly won"

Select a one-on-one Extreme Rules match (can be any match, but I chose that one). Then select two wrestlers. Hit cn_Y to go to match options, select title match and then an available title. Hit cn_start twice to start the match.

In the top left corner of the screen you'll see hit cn_A for a break out. Under that you'll find options for weapons Use the D-Pad to scroll to Championship and hit cn_A to break out. Now you can beat down your opponent with the title and get the achievement.

For this, pick anyone for yourself and pick anyone who currently holds a championship in your game as your opponent. Make sure entrances are on. Don’t skip your opponent’s entrance. Wait until either Run In or Break Out appears in the top left (depending on the order you entered), then do so. Run towards your opponent and he should drop his belt. You can now pick it up with before hitting him with it for the trophy.
