Worth Having to Re-Climb - Perform your finisher off the top of the ladder.
Make sure your Momentum is flashing when both you and a opponent are at the top of a ladder stun them then press Y.
Each character will have a different one - Christian's for example is called the Killswitch, basicly it's his normal finisher but off a ladder.

i haven't tried this but you should be able to get Please Don't Try This at Home - Push an opponent off a ladder, so they fall out of the ring and through a table at the same time as this use randy ortons rko through a table off the ladder just a thought.

This is easier in a one on one ladder or TLC match, Get a signature move and climb up one side of a ladder so your opponent should climb up the other side. Hit your opponent a few times to stun him and get the finisher then press 
to execute your ladder finisher and the trophy will pop immediately after landing.
