I can't remember the name of the level, but the passage when the blades hanged to the ceiling drops down on you. The game will give you a guide on how to perform a Roll.
Try and past that passage without the blades dropping down on you and you'll get the achievement.

This is the most easy level to get this achievement. Once you figure out how to roll it's not that hard (forward, forward + A in rapid succesion). I also recommend saving after each guillotine. Name of the level is Deadly Passage.

There is a level called "Blades of Death" where all you have to do is run across this small rotating pole that has some blades on them. Im not 100% sure if the blades can damage you but i didnt touch them and the achievement unlocked right after i finished the level. oh and there are also no monsters, valuables, or artifacts on this lvl.
