How to unlock the This can't be happening! achievement in X-Men: Destiny - Definitive Guide
Equiping this x-gene and upgrading it so you stun enemies as your run past them will afford you this achievement rather easily. The stun apparently counts toward a combo. You can get this in several seconds with just a few number of enemies around. Just run around(hold rt+a) and stun guys until the "insane combo" text shows up.
I got this in the warehouse for Gambit's mission a couple hours in. You will find yourself surrounded by about a dozen enemies. Use the Shadow Tornado power (your first Mutant Power as a Shadow mutant) and it will pick them up all around you. Got both the Great Combo and the Incredible Combo at the same time.
Near the end of Level 7, there will be a few areas with a large number of enemies. If you have chosen the X-Men side, you will accompany a mech that Forge has hacked. There will be more than 150 enemies that will spawn during this part of the level. Once you have enough energy to become the Obsidian Titan, transform and attack. Your ground-pounding powers will instantly kill multiple enemies and rack up quick combos. In one turn as the Obsidian Titan, I managed two Insane combos, plus one more after another transformation.
Shortly after that, there is an area that has infinitely spawning enemies. You are supposed to head up some stairs while your teammates (regardless of which side you chose) hold them off. An Obsidian Titan transformation here garnered me another couple of Insane combos. (This is also a really good place to grind the Goon Squad achievement for killing 500 MRD troops. I did my final 200 in ten or fifteen minutes).
Immediately after that, once you head up the stairs, there will be a narrow path along which a number of the mutated MRD troops (the guys with the things sticking out of their bodies) will spawn. You can go either with the Obsidian Titan (just start smashing) or the Boulder Dash (just start rolling). Either one will net you an Insane combo.
And of course, if you wait until the end of the game there's that large arena full of enemies that you fight during the credits. One transformation into the Obsidian Titan should do the trick.

I've put together a few clips showcasing each of these powers in action and pulling off Insane Combos. It's really easy to do once you have the right power.
There's really no way to see exactly how many combos your mutant has performed. However, when fighting large groups of enemies you'll see Combat Text Pop Ups. You'll naturally end up performing an insane combo just by fighting a large number of enemies at once. Eventually you'll see the Combat Text Pop Up of: INSANE COMBO pop up on your screen. When this happened for the first time your trophy will pop. If you completed the game and you still have not unlocked this trophy yet there are a few different ways to unlock it. Once you've completed the game and the credits begins to roll, you’ll have full control over your mutant character and will be able to fight over 200 Purifiers while the credits are still rolling. This is an excellent opportunity to obtain the trophy for there are many enemies for you to defeat. You can also simply use your Ultra power by pressing both the + buttons and then attack a group of enemies with the button. Your Ultra power can easily rank up combos and other Combat Text Pop Ups.
You can get the trophy in the usual way. Try not to repeat your attacks, use superpowers (L+ , + ), block before getting hit and dodge. If you get hit, the combo will break.
