How to unlock the A Toast Together achievement in Yakuza 5 Remastered - Definitive Guide
You can start this achievement as early as Part 1. However, it can't end until the final chapter. At any point once Akiyama reaches Kamurocho during Finale, Chapter 3, go to Elise to start this sidequest (the game doesn't inform you about it). Then, during the Final Chapter (or do the entire thing during Premium Adventure if you want), select Akiyama while at New Serena.
He will tell the protagonists to return to the parts of the game they're familiar with (each character is able to go to their own town) and recruit Hostesses. If you've been staying with it (S rank is required), you should only need to speak to them once more.
Once you've recruited four hostesses, Kiryu can recruit the fifth. Once that's settled, return to Elise as Akiyama for the final toast and the achievement.
NOTE 1: In order to visit a club, you first need to visit an adult entertainment info booth. You can't just pass on it. You need to actually be escorted to it.
NOTE 2: For your order, I recommend the Gatcha drink. Not only does the meter go up, but you also get something in return for the order. Furthermore, if you land the Platinum Business Card, you can equip it and get 30% off your cover charge each visit to save money.
NOTE 3: You can also order the Yamazaki 12 years, which will save you money because you can keep it as long as it's not empty, but you're better off ordering the free Kyogetsu Green for the slight affection boost since the Yamazaki 12 Years only gives you the boost upon ordering it. Also, the more you spend, the bigger the boost, though anything above the Gatcha Drink is a waist of money.
NOTE 4: For the S Rank visit (you need to do S Rank before you can recruit them), you can leave at your first opportunity, even if you don't have the affection meter maxed out. I know because I tested it. Then you can recruit them.
NOTE 5: You need ¥5 million on hand before you do the toast.

For the final event, you have to enter Elise as Akiyama to trigger it. If you walk in as Kiryu it'll just give you the generic would you like to request a girl dialogue.

Yeah, I read Kiryu on another guide for some reason. I fixed it.

This trophy is awarded after 100% completion of all five Hostess story lines, plus a toast to Club Elise.
Finishing the first four hostesses is pretty straightforward, but the fifth hostess, Hinata Osaki, is tricky.
1. In the Finale, or Premium Adventure, visit Elise as Akiyama. This starts a story line about finding hostesses from other cities.
2. Each character must now visit the hostess club that they had their hostess story line with, and invite them to come to Elise in Kamurocho.
3. As Akiyama, return to Elise and you will be introduced to Hinata.
4. As Kiryu, complete the entire Hinata story line. In order to fully finish this, you will need 5 million yen on hand at the end. You don't need to spend it, you just need to have it.
5. Finally -- and this is the part that is very easy to miss, since there is no prompt to do it -- visit Elise one more time with Akiyama to trigger the toast.
You could also use this opportunity to raise a toast to Kiryu for showing remarkable self-restraint this time around and limiting his hostess "activities" to just two ladies. Okay, three if you count his hostess-girlfriend. But don't worry, he'll be back up to his usual cavalcade of ladies in Yakuza 6.
