After you complete the C Team's Rec Room quest, you'll be tasked with rolling three dice with the goal of rolling all ones. This is completely controlled by the game, there's nothing you can do to effect the outcome.
The first time you roll you will fail and die, getting a bad ending. Retry and you'll die again. Retrying a third time you will roll the three ones and unlock the achievement, as well as a different ending to progress the story.
