After you complete the D Team's quest, "Trash Disposal Room", you'll be tasked with shooting a gun or not. You'll have to do the scenario multiple times as there's three endings related to it for a "see all possible endings achievement":
- Do nothing, let the timer run out
- Pull the trigger, the bullet is real
- Pull the trigger, the bullet is fake - this one will unlock the achievement. It's randomly generated if the bullet is real or not, so keep replaying this scenario until you get both outcomes
