After you complete the D-Team's mission "Healing Room", you will find a large, beautiful blue button just BEGGING to be pressed.
A timer of 64 seconds will pop up, simply avoid all temptation and let this timer count down to zero without pressing that tantalizing button in order to unlock the achievement.
There's also an achievement for seeing all possible endings in the game, so you'll have to play this scenario again and push the button.
