Ok, so you can get this on level 2, when you get to the hotel.
In order to poison her, what you ultimately have to do is poison the cake in the kitchen on floor 1, and then send it up to her via the dumb waiter (also on floor 1). As such key here is DO NOT KILL any of the staff on the first floor (2 cooks and a waiter)
When I first went to floor one, I didn't see a cake in the kitchen, and as such, just kept exploring. On floor two there is some poison you can pick up in the storeroom at the back (the key to open this room is in the lift you can open with the access card). Go all the way up to floor 5, so you see the fat bitch in her room (but don't fight her). She will move to the back room, allowing you to access her room and floor 6.
When I then went down to floor 1, the cooks had made a cake on the table. What I did was equip the poison and just walk up to the cake and touch it (don't pick it up). It changed colour, indicating it was poisoned, and I then walked away. The waiter will then put it in the dumb waiter for you. I would suggest you save just before this point, as a couple of times when trying I picked up the cake, which I then dropped and ruined trying to get it in the dumb waiter - so if this happens to you, juts reload the checkpoint.
When it goes in the dumb waiter, return to floor 5 and watch the fat bitch die!! Good luck!!
