These events beings:

Green Liquid (Eventually a green liquid will drip from the the cieling revealing an item event (Its 3 items, 2 of them being: Gas Mask, and Survival Book). Don't use anything (this might also make some people sick) and hope you got her!)
The % is apparently pretty small like 0.5% - 2%
So keep and eye out for the chances and if your able to take them!

I wasn't even going for this when it unlocked but it's weird why this even happened on the medium difficulty and not on easy. It was either waste the gas mask on the green liquid or do nothing and risk losing a crazy but not sick Mary Jane. I was actually trying to setup the Military ending so I can finally clear all modes on medium difficulty as for some reason getting the twins wasn't counting for that achievement.
